Upon his passing, Mr. Wells  wished to promote Community Health causes and, through his will, established a fund to award grants in support of health among other areas.

Successful awardees will demonstrate that their application qualifies under the terms of that portion of Mr. Fred Wells' will which stated the awards were to be used for “the care of the sick or aged by providing home, nurse, medicine or hospital care” and “for helping to provide medical or nurses care in rural districts where no such facilities are to be had.”

The application, along with two complete copies of all required material, must be submitted before the deadline posted on the application. Decisions on Grants will be made in May and notification will be made to successful applicants.
